Case Manager

Our client, a leader in healthcare services, is seeking a Case Manager to join their team. As a Case Manager, you will be part of a multidisciplinary team supporting individual and small group members. The ideal candidate will have strong communication skills, a collaborative mindset, and a commitment to patient-centered care, which will align successfully in the organization.

Location: Durham, NC (Remote)

Pay Range:

$41/hr on W2

What's the Job?

Serve as a team member on a multidisciplinary team, coordinating care, resources, and services for members to achieve optimal clinical and resource outcomes.

Utilize applicable clinical skillset and perform comprehensive assessments to determine how to best collaborate with members, family, internal partners, and external services/providers on plans for treatment.

Develop a member-centric plan tailored to members? needs, health status, educational status, and level of support needs.

Perform ongoing monitoring and management of members, including scheduled follow-ups and assessment of progress towards goals.

Educate members and encourage proactive intervention to limit expenses and promote positive outcomes.

What's Needed?

RN, RD, or LCSW with 3 years of clinical and/or case management experience required.

Must have and maintain a valid and applicable clinical license (NC or compact multi-state licensure).

Experience in MS Office and electronic documentation systems is required.

Hospital background or case management experience is helpful.

Ability to effectively document all aspects of the care plan and member interactions.
