What does a Case Manager do?
A case manager is someone who helps people with their healthcare and social service needs by connecting them with the necessary services. This can include individuals who are chronically ill, elderly, children, or going through physical rehabilitation.
Job Description
We are looking for a new case manager to join our team and take on various new and existing cases. The ideal candidate will have previous experience as a case manager and be able to handle a large portfolio of cases. A bachelor's degree in a related field and experience working with clients to develop treatment plans are required.
Job Duties and Responsibilities
- Manage a portfolio of cases
- Regularly meet with clients
- Build and maintain relationships with key stakeholders
- Proactively coordinate with service providers
- Follow relevant legislation, best practices, and internal policies and procedures
- Accurately update and maintain case information
- Create and maintain treatment plans with clients
- Collaborate with service providers on discharge plans
Requirements and Qualifications
- Bachelor's or master's degree in business, communications, social work, or related field
- Previous experience as a case manager
- Experience developing treatment plans with clients
- Knowledge of relevant legislation
- Valid driver's license
- Background in healthcare, social work, mental health, disability care, substance abuse recovery, geriatric care, or long-term care
- Customer service experience
- Excellent communication skills
- Ability to manage multiple tasks and prioritize effectively
