Job Description
We are looking for a dedicated RN case manager to coordinate and advocate for our patients' care. This role involves reviewing patient charts, meeting with families and healthcare providers, and staying in touch with patients throughout their treatment process.
The successful candidate will be able to remove logistical barriers for patients, allowing them to focus on their health. They will also build strong relationships with patients and their families.
Job Duties and Responsibilities
- Conduct intake assessments with patients and families
- Ensure families feel supported and equipped
- Create individualized treatment plans that consider physical, financial, and other constraints
- Adjust treatment plans as needed
- Coordinate follow-up services to promote wellness
- Arrange transportation for patients to and from healthcare facilities
- Meet with healthcare team members to discuss patient progress and determine appropriate care
- Check for insurance coverage or financial resources to pay for treatment
- Find solutions or referrals for patients who cannot afford treatment
Requirements and Qualifications
- Bachelor's degree in nursing
- Advanced degree in nursing is preferred
- Certificate in community and/or hospital case management
- Prior experience as an RN case manager in a similar healthcare setting
- Valid driver's license and personal vehicle
- Familiarity with patient databases
- Strong understanding of patient information confidentiality
- Excellent leadership, supervision, and motivational skills

- Ability to work evenings, weekends, and holidays
