Job Description
We are seeking a dynamic clinical case manager to join our medical facility. In this role, you will be responsible for evaluating patients' treatment plans, monitoring their well-being, creating progress reports, and ensuring they are informed about their care and treatment options.

Job Duties and Responsibilities
- Admitting patients to the medical facility, reviewing records, and conducting orientation.
- Coordinating and providing patient care based on an approved treatment plan.
- Facilitating aspects of the treatment plan, such as care coordination and discussions with the patient's family.
- Recording all case information, including care procedures, appointments, and check-ups.
- Arranging for the intervention of social services, health, or governmental agencies when necessary.
- Evaluating patients' progress and determining the need for additional evaluations.
- Interacting with patients to ensure they are informed about their treatment plan and answering any questions they may have.
- Writing case reports to determine when a case can be closed.
- Building strong relationships with the medical team during the case management process.
Requirements and Qualifications
- A bachelor's degree in nursing or a related healthcare field.
- A healthcare-related license, certification, or registration is preferred.
- At least 3 years of experience as a clinical case manager or in a similar healthcare role.
- Excellent knowledge of case management principles and healthcare management.
- Ability to work with patients with disabilities, mental illnesses, or challenging behaviors.
- Strong communication and interpersonal skills.
- Compassion, empathy, patience, and calmness when working with patients.
- Organizational skills and attention to detail.
