Job Description

Schedule:

Shift: Mid-shift, 4x10-hour shifts (1:00 PM - 11:00 PM), every other weekend required

Duration: 13 weeks

Responsibilities:

Provide case management for patients in a Level 1 trauma center Emergency Department

Coordinate care for acute inpatient cases, ensuring efficient and quality outcomes

Utilize EPIC documentation system for accurate record-keeping

Collaborate with multidisciplinary teams for discharge planning and patient advocacy

Requirements

Qualifications:

Experience: Minimum of 3 years of acute inpatient case management experience in a Level 1 Trauma Center

Certifications: BLS (Basic Life Support) certification required

First-timers: Accepted

Requirements:

Flu vaccine required (medical/religious exemptions accepted)

COVID-19 vaccine not required
