Position Overview:

We are seeking a Board-Certified, Psychiatric Nurse Practitioner to provide high-quality medical care through home visits in Bronx. NY. This unique role offers the opportunity to deliver personalized care directly to patients in their homes, with full support including a personal driver and medical scribe .

Qualifications:

• Education: Master's degree in Nursing (MSN)
• Licensure: Active NY State NP License (required)
• Certification: Board Certification required
• Experience: Must have at least 1 year of experience working as a PMHNP
• Technical Skills: Proficiency in physical assessments, diagnostic reasoning, and treatment planning
• Soft Skills: Excellent communication, adaptability, and compassion for homebound patients

Key Responsibilities:

• Conduct comprehensive in-home physical examinations
• Document medical histories and symptoms
• Develop and implement individualized care plans
• Order and interpret diagnostic tests
• Prescribe medications and therapies
• Provide patient and family education on disease management
• Coordinate care with physicians and specialists
• Utilize the provided scribe for efficient documentation
