Position Overview:
Greenlife Healthcare Staffing is seeking a compassionate Family orAdult Geriatric Nurse Practitioner (NP) to join a nursing home in Brooklyn, NY. - Conduct comprehensive patient assessments, including medical history reviews and physical exams.
- Develop and update individualized care plans for residents with acute/chronic conditions.
- Monitor and manage infections by adhering to facility protocols.
- Educate patients and families on disease management, prevention, and wellness.
- Collaborate with physicians, nurses, and therapists to ensure coordinated care.
- Maintain accurate documentation of treatments, progress, and care plans.
