Job Description
As a Hospice Nurse Case Manager, you'll do more than provide medical careyou'll offer emotional support, guidance, and reassurance to individuals facing terminal illness. Your role will be pivotal in ensuring patients experience the highest quality of life in their final days, surrounded by respect, compassion, and dignity.
What You'll Do
- Coordinate Compassionate Care : Schedule patient visits according to individual needs, collaborate with physicians, manage orders, and oversee medication administration.
- Lead and Support Teams : Supervise LPNs, LVNs, and Hospice Aides, ensuring consistent, high-quality care.
- Holistic Approach : Deliver comprehensive palliative care that addresses the physical, emotional, and spiritual needs of patients and their families.
- Guide Families : Provide crucial emotional support and guidance, helping families navigate challenging decisions with compassion and clarity.
Requirements
Associate's degree preferred
Active Registered Nurse license required
Minimum of one year of nursing experience
Previous experience in hospice care strongly preferred
Willingness to serve on-call as needed
Reliable transportation and a valid driver's license
