Develop and implement personalized care plans tailored to patients' needs.
Coordinate and manage patient visits, ensuring seamless care delivery.
Oversee medication administration and timely order management.
Supervise and support LPNs, LVNs, and Hospice Aides.
Work closely with physicians and interdisciplinary teams to provide holistic care.
Educate and support families, equipping them with resources and guidance.
Uphold the highest standards of empathy, dignity, and integrity in all interactions.
Requirements
What We're Looking For: Registered Nurse (RN) license required (Associate's degree preferred).
Minimum of one year of nursing experience (hospice experience a plus!).
Compassionate, patient-centered approach to care.
Willingness to participate in on-call rotations .
Reliable transportation and a valid driver's license.
