Job Description

Title: Masters Level Social Worker

Summary:

The Master Social Worker provides counseling, psychosocial evaluation, and resource referral services to patients at our facilities, and remotely. They collaborate with the treatment team, advocate for patient needs, and maintain accurate documentation.

Responsibilities:

Conduct comprehensive psychosocial assessments and provide counseling services to patients and their families

Offer case management and coordinate services to optimize patient functioning

Refer patients to community services and work with the interdisciplinary team for care planning

Develop resources to meet patient needs and participate in orientation, education, and rehabilitation programs

Advocate for patients within the renal setting and participate in professional social work organizations

Professionalism:

Maintains confidentiality and adheres to employee policies

Demonstrates adaptability and works harmoniously with others

Demonstrates dependability and completes job duties as assigned

Teamwork and Communication:

Works cooperatively with other team members

Strives to use clear verbal communication with patients and demonstrates good listening skills
