Job Description

We are seeking a licensed clinical social worker (LCSW) or licensed master social worker (LMSW) to join our team as a Case Manager. The ideal candidate will be responsible for providing comprehensive case management services to individuals and families in need.

Responsibilities:

Conduct initial assessments to determine clients' needs and develop individualized care plans

Provide counseling and support services to clients dealing with mental health and/or substance abuse issues

Coordinate with external agencies and service providers to ensure clients receive the necessary support and resources

Monitor client progress and make appropriate referrals as needed

Maintain accurate and up-to-date case records and documentation

Collaborate with a multidisciplinary team to ensure the overall well-being of clients

Qualifications:

Master's degree in social work

LCSW or LMSW license in the state of employment

Experience in case management, preferably in a healthcare or social services setting

Strong communication and interpersonal skills

Ability to work effectively in a fast-paced and dynamic environment

Understanding of and commitment to ethical practices and confidentiality
