Job Description
Expert Healthcare is seeking a compassionate and skilled Home Health Physical Therapist to join our dedicated team. In this role, you will provide personalized rehabilitative care to patients in the comfort of their own homes. Your expertise will be essential in developing individualized treatment plans aimed at restoring mobility, improving strength, and enhancing overall quality of life after illness or injury. You will conduct thorough evaluations, set realistic goals, and collaborate with other healthcare professionals to ensure comprehensive patient care. Schedule: M-F day shift, 1 weekend/month
Coverage Area: Merced County
Productivity: Minimum 30 points per 40 hour week
Responsibilities- Perform evaluations and develop tailored treatment plans based on patient needs.
- Implement therapeutic exercises and interventions to facilitate recovery.
- Monitor patient progress and modify treatment plans as necessary.
- Educate patients and their families on rehabilitation exercises and equipment usage.
- Coordinate with other healthcare providers to ensure optimal patient care.
- Document patient care and progress in compliance with regulatory standards.
Requirements
- Current and valid Physical Therapist license in the state of practice.
- Degree in Physical Therapy from an accredited program.
- Must have 12 months of Home Health experience within the previous 3 years.
- Must have HomeCare HomeBase Charting Experience
- Strong communication and interpersonal skills.
- Ability to work independently and manage a caseload.
- Basic life support (BLS) certification is required.
