Job Description
- Conduct an admission interview with the resident and family for the purpose of completing a confidential psychosocial history and assessment, identify areas of concern, develop treatment plans/goals, and complete advance directives.
- Update residents' charts/social services records every three months following initial evaluation and/or as social services needs are demonstrated. Re-evaluate resident care plans at least every three months.
- Assisting with scheduling transportation
- Send bereavementcards and notifyfamilies or responsiblepartieswhen residents expire and document
- Contact families to pick up personal belonging for resident who return to the hospital
- Handleancillary services for the residents (dental, ent, optometry, psychology consult, ophthalmology)
- Oversee room changes - completing acknowledgement form, document, and and notify families.
- Attend weight and wounds meetings and complete the social service section of assessment for weight variance meeting
- Work on the weekends if able
- Theft & loss and grievances
- Conduct Angel rounds & IP rounds
- Assist with discharge planning
- Contribute to Resident Care Conferences, and communicate resident and family areas of concern.

Social Services Designee Certificate
Prior Skilled Nursing Experience
Ability to speak and write English well
