Registered Nurse, RN

We are looking for a compassionate and dedicated Home Health Registered Nurse (RN) to join our team, delivering top-notch, personalized care to patients in the comfort of their own homes. In this role, you'll conduct in-home assessments, develop and adjust personalized care plans, and deliver skilled nursing treatments as prescribed. You'll also educate patients and caregivers on managing health conditions and provide emotional support through each step of their recovery. Working closely with a network of healthcare professionals, you'll help to ensure that each patient receives coordinated, comprehensive care that leads to optimal health outcomes.

• Patient Assessments : Perform thorough assessments to evaluate each patient's health status and determine their unique care needs.
• Care Plan Development : Collaborate with patients and their families to develop and continually refine individualized care plans.
• Treatment Administration : Administer prescribed medications and treatments accurately and safely.
• Patient Monitoring : Track and document patient progress, and adjust care plans as necessary based on clinical findings.
• Health Education : Empower patients and caregivers by providing clear guidance on managing conditions, medication adherence, and health maintenance.
• Coordination of Care : Partner with interdisciplinary healthcare teams to ensure patients receive comprehensive, well-rounded care.
• Timely Patient Support : Address patient inquiries, provide prompt support, and facilitate positive patient experiences.

Requirements

Licensing : Active, unrestricted RN license in the State of California

Experience : Minimum of 1 year in Home Health or Acute Care nursing

Additional Requirements : Valid driver's license with a clean DMV record
