We are seeking a dedicated Primary Care Physician (PCP) to join a dynamic multi-specialty practice in Queens, Bronx, and Brooklyn, NY . This is an exciting opportunity to provide comprehensive, integrative care in a mission-driven setting focused on innovation, wellness, and community health.

• Deliver comprehensive primary care including diagnosis, treatment, and chronic disease management
• Coordinate care with specialists and allied health professionals
• Provide preventive health education and lifestyle counseling
• Maintain timely and accurate patient documentation
• Participate in quality initiatives and continuous care improvements

Requirements

• Education: MD or DO from an accredited institution
• Licensure/Certifications:
• Active NY State medical license
• Board Certified/Eligible in Internal Medicine, Family Medicine, or Primary Care
• Experience: All experience levels welcome
• Language Requirement: Bilingual in Spanish required
• Technical Skills: EHR proficiency, chronic care management, and preventive medicine
• Soft Skills: Strong communication, cultural competence, and teamwork mindset
