Job Description
We are seeking a dedicated and experienced Primary Care Nurse Practitioner - AGNP to join our healthcare team. The ideal candidate will provide comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and the management of common acute and chronic illnesses.
Responsibilities:
Conduct comprehensive assessments and develop individualized treatment plans for patients
Order and interpret diagnostic tests
Prescribe medications and other treatments
Provide patient education and counseling on health maintenance and disease prevention
Collaborate with physicians and other healthcare professionals to ensure coordinated and integrated care
Maintain accurate and up-to-date medical records
Adhere to all regulatory and ethical standards in healthcare delivery
Qualifications:
Master's or Doctoral degree in nursing
Current certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGNP)
Valid state nurse practitioner license
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Ability to work effectively in a collaborative healthcare team
