This role is open to both new graduates and experienced providers , and can be full-time or part-time based on your availability.

• Deliver comprehensive medical care to patients in nursing home environments
• Perform physical exams, assess patient health, and develop treatment plans
• Prescribe medications and therapeutic interventions as needed
• Coordinate care with physicians, nurses, and interdisciplinary teams
• Educate patients and their families on health management and preventive care
• Maintain accurate and timely medical records

Requirements

• Education: Master's degree in Nursing (MSN) from an accredited program
• Licensure: Active New York State Nurse Practitioner license & Board Certification
• Experience: New graduates encouraged to apply
• Technical Skills: Proficient in eClinicalWorks EMR
• Soft Skills: Strong communication, autonomy, and collaborative mindset
