Job Description


  • Nurse Navigator LPN or RN ComprehensiveDisease Management (Palliative Care)
  • Full Time; Monday through Friday 8am - 5pm
  • Topeka, KS and surrounding areas
  • Pay Range: $60,000-70,000

The Comprehensive Disease Management (CDM)/Palliative Nurse Navigator helps patients navigate their healthcare journey by having a goals of care conversation, identifying their healthcare needs, recommending resources, communicating with patient's healthcare providers and educating on disease progression and treatment/care options available. In an overwhelming time where patients and families need to make critical healthcare decisions, CDM Nurse Navigators are able to walk this journey with people so they can confidently make healthcare decisions that align with their personal goals.

Responsibilities

  • Connecting with patients and establishing consistent face to face encounters
  • Establishing rapport with patients and initiating goals of care conversations
  • Educating on the benefits and goals of the various homecare services and making appropriate referrals to those services when patient desires
  • Provide assistance with advance care planning and disease specific advance directives
  • Navigate patients through various care planning tasks such as: disease progression and goals of care conversations, coordination with specialistsand connection to available community resources
  • Completing accurate and professional documentation after each visit
  • Building and maintaining strong partnerships with community healthcare partners
  • Connecting and communicating at a high level with Phoenix colleagues to ensure continuity and collaboration of care.

Requirements

  • Must have an active Kansas or Compact RN or LPN license.
  • Must have active driver's license.
  • Hospice or Home Health experience preferred.
