We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join a skilled nursing facility in Queens, NY . This full-time role involves delivering high-quality, resident-centered care, adhering to established care plans, and working collaboratively with interdisciplinary teams to promote optimal health outcomes.

• Administer prescribed medications and treatments
• Monitor, record, and report changes in residents' vital signs and health conditions
• Collaborate with RNs, physicians, and other healthcare providers to execute care plans
• Assist residents with daily activities, including bathing, dressing, and mobility
• Uphold infection control protocols and facility policies
• Provide timely responses during medical emergencies
• Maintain clear, accurate, and timely documentation in patient records

Requirements

• Education: Completion of an accredited Licensed Practical Nursing (LPN) program
• License: Active New York State LPN license
• Immunization: Proof of Hepatitis B immunity required
• Experience: Minimum 1 year of experience in a long-term care or skilled nursing setting
• Skills:
• Strong communication and teamwork abilities
• Adaptability to rotating shifts and fast-paced environments
