• Conduct vital sign assessments and patient evaluations
• Administer medications, wound care, and other clinical treatments
• Coordinate with RNs, MDs, and rehab staff on care plans
• Accurately document patient care and changes in condition
• Support patient independence and recovery goals
• Adhere to infection control and facility policies

Requirements

• Graduate of an accredited LPN program
• Active MA LPN license and BLS certification
• At least 1 year of LPN experience in long-term care, SNF, or rehab
• Proficient in medication administration , wound care , and EHR documentation
• Compassionate, detail-oriented, and team-focused
