Greenlife Healthcare Staffing is hiring a Licensed Practical Nurse (LPN) for a per-diem day shift at a Skilled Nursing and Rehabilitation Facility in Holyoke, MA . - Conduct vital sign assessments and patient evaluations
- Administer medications, wound care, and other clinical treatments
- Coordinate with RNs, MDs, and rehab staff on care plans
- Accurately document patient care and changes in condition
- Support patient independence and recovery goals
- Adhere to infection control and facility policies

Requirements
Qualifications- Graduate of an accredited LPN program
- Active MA LPN license and BLS certification
- At least 1 year of LPN experience in long-term care, SNF, or rehab
- Proficient in medication administration , wound care , and EHR documentation
- Compassionate, detail-oriented, and team-focused
