Monday - Friday, No Weekends, No Call required.
Key Responsibilities- Perform comprehensive assessments of patients in their home environments.
- Design, implement, and modify individualized care plans in collaboration with the healthcare team.
- Administer prescribed medications and treatments appropriately and safely.
- Monitor and document patients' progress, making adjustments as necessary.
- Provide education to patients and families on health conditions and care plans.
- Facilitate communication and coordination among team members and other healthcare providers.
- Offer emotional support and guidance to patients and families throughout the care process.
Requirements
- Current RN license in the state of practice.
- Experience with HomeCare Homebase Charting required.
- Strong clinical assessment skills and critical thinking capabilities.
- Excellent communication skills and the ability to build rapport with patients.
- Self-motivated with strong time management skills.
- BLS certification required.
- OASIS experience required.
- Must have at least 12 months of home health experience within the past 3 years.
- Must meet minimum productivity expectation of 30 points per week.
