• Conduct thorough assessments of patients' health status in their home environment.
• Develop and implement individualized care plans based on patient needs and goals.
• Administer medications and treatments as prescribed by physicians.
• Monitor patients' progress and maintain accurate medical records.
• Educate patients and their families about health management and care practices.
• Coordinate with interdisciplinary teams to provide holistic patient care.
• Provide emotional support and guidance to patients and their families.

Requirements

• Current and valid RN license in the state of practice.
• HomeCare Homebase Charting Experience Required
• Minimum of 1 years of direct home health experience, within the last 3 years.
• Strong clinical assessment and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Basic life support (BLS) certification required.
