Home Health Physical Therapist

Monday - Friday, 40 hours per week. Must meet minimum productivity expectations.

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Deliver effective therapeutic exercises tailored to patient needs.
• Track and record patient progress, adjusting treatments as necessary.
• Educate patients and families on rehabilitation practices and equipment.
• Collaborate with multidisciplinary teams to enhance patient outcomes.
• Maintain accurate records in compliance with healthcare regulations.

Requirements

• Valid Physical Therapist license in the state of practice.
• Bachelor's or Doctorate in Physical Therapy from an accredited institution.
• Experience in Home Health setting preferred.
• HomeCare HomeBase charting experience preferred.
• Exceptional communication and interpersonal skills.
• Ability to work autonomously and manage a diverse caseload.
• BLS certification is required.
