Monday - Friday, 40 hours per week. Must meet minimum productivity expectations.
Key Responsibilities- Conduct comprehensive evaluations and develop individualized treatment plans.
- Deliver effective therapeutic exercises tailored to patient needs.
- Track and record patient progress, adjusting treatments as necessary.
- Educate patients and families on rehabilitation practices and equipment.
- Collaborate with multidisciplinary teams to enhance patient outcomes.
- Maintain accurate records in compliance with healthcare regulations.
Requirements
- Valid Physical Therapist license in the state of practice.
- Bachelor's or Doctorate in Physical Therapy from an accredited institution.
- Experience in Home Health setting preferred.
- HomeCare HomeBase charting experience preferred.
- Exceptional communication and interpersonal skills.
- Ability to work autonomously and manage a diverse caseload.
- BLS certification is required.
