Schedule: Monday Through Friday
Responsibilities- Conduct comprehensive assessments of patients' physical, emotional, and cognitive abilities.
- Develop and implement individualized treatment plans in collaboration with other healthcare professionals.
- Provide therapy sessions in patients' homes, adjusting interventions based on their specific needs and progress.
- Educate patients and their families about therapeutic activities and strategies to enhance daily living skills.
- Document patient evaluations, progress, and outcomes in compliance with applicable regulations.
- Modify treatment plans as necessary and communicate updates to relevant team members.
Requirements
- Current and valid Occupational Therapist license in the state of practice.
- Graduation from an accredited Occupational Therapy program.
- Experience in home health setting required.
- HomeCare HomeBase Charting Experience Required
- Strong assessment, planning, and therapeutic skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a healthcare team.
- Basic Life Support (BLS) certification required.
