Care Coordinator

Mental Health Association is hiring for a Care Coordinator. The Care Coordinator is responsible for assisting clients to obtain needed services by providing information, referral coordination and follow-up. Developing and connecting with resources and behavioral services offered through health insurance. Additionally, the Care coordinator will coordinate Medication Assisted Treatment and psychiatric services while resident is in the program as well as assisting in the development of aftercare plans.

Regularly attend and actively participate in team meetings, supervision meetings and/or other scheduled meetings. Attend agency mandated training and maintain required certifications. Pursue additional activities and training to enhance job knowledge and competency which afford opportunities for professional growth.Maintain and foster a team atmosphere with colleagues and Supervisor with participant needs as the priority. Maintain positive attitude with team and participants.Use constructive supervisor feedback to grow and enhance professional skills.

Requirements

• Must have High School Diploma or equivalent preferred (GED)
• Bachelor's Degree preferred
• Must have Human Service experience
