Join our passionate team at MHA and play a vital role in supporting young adult males (ages 1824) living with co-occurring substance use and mental health conditions. Ridgewood Terrace, one of our GRIT programs, offers a 24-hour structured, therapeutic residential setting where residents work toward recovery and stability.

As a Care Coordinator you will be responsible for helping individuals connect with vital services, behavioral health resources, and aftercare support. You'll ensure smooth coordination of care, from Medication Assisted Treatment and psychiatric services to post-discharge planning.

Schedule: Mon-Thurs, & Sundays (Full-time, 40 hours/week, daytime hours with flexibility) What You'll Do:
  • Assist residents in accessing behavioral health, medical, and community-based services.
  • Coordinate referrals and follow-ups for ongoing care and treatment needs.
  • Support the development and implementation of aftercare/discharge plans.
  • Build relationships with providers, health insurers, and community supports.
  • Promote a safe, respectful, and inclusive program environment.

Requirements

  • Required: High School Diploma or GED.
  • Preferred: Bachelor's Degree in Human Services or related field.
  • Experience working in a human services, mental health, or substance use recovery setting.
  • Strong communication, organization, and collaboration skills.
  • A positive, team-oriented mindset and a passion for empowering others.
Working Conditions & Physical Demands:
  • Work is primarily indoors, with occasional travel and exposure to varied weather.
  • Requires sitting, typing, speaking, and occasional lifting (up to 40 lbs).
