What does a Continuous Care LVN do?

We are looking for a skilled Continuous Care LVN to monitor our patients and report any changes in their condition. Your responsibilities will include administering treatment under the supervision of registered nurses, updating patient files, and educating families on patient care.

Job Duties and Responsibilities
- Provide nursing services to patients under the guidance of registered nurses.
- Travel to patient locations to provide bedside care.
- Evaluate treatment plans and coordinate patient care with other healthcare professionals.
- Take and record vital signs, such as blood pressure, heart rate, and pulse oximetry.
- Administer pain or symptom management medication as needed.
- Assess patient response to treatments and medications and take appropriate action when necessary.
- Educate families and caregivers on patient care and address any concerns sensitively.
- Maintain and update patient files and medical records.
- Perform other tasks assigned by the registered nurse.
- Follow best practices in nursing and continue to develop professionally.
Requirements and Qualifications
- Must be a licensed LVN/LPN in the state.
- Certification in CPR and IV therapy is a plus.
- 2-3 years of experience as a continuous care LVN or in a similar role.
- Proficiency in electronic health record management systems, such as Axxess Hospice and gEHRiMed.
- Ability to follow treatment instructions, collaborate on patient care, and accurately document processes.
- Advanced skills in evaluating and treating patients and implementing necessary interventions.
- Experience in providing patient care information and support to families.
- Willingness and ability to travel to patient locations.
- In-depth knowledge of best practices in providing continuous nursing care.
- Excellent interpersonal skills to support patients and their families.
