Social Worker

We are seeking a dedicated and compassionate Social Worker in Washington, DC. The ideal candidate will possess a strong background providing essential support to individuals.

Required: Care Coordination, Discharge Planning, Discharge Planning, Needs Assessment/ Order DME, Plan of Care Collaborative care, Coordinate services to home care, Documents pertinent information to facilitate care coordination, participates in care conference activities, Provide crisis intervention

Responsibilities

• Conduct thorough assessments of clients personalized treatment plans.
• Maintain accurate medical documentation and records in compliance with HIPAA standards.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care and continuity of services.
• Monitor client progress and adjust treatment plans as necessary based on ongoing evaluations.

Requirements

• Master's degree in Social Work (MSW) from an accredited institution
• LPC or LGPC
• Strong organizational skills with attention to detail for accurate medical documentation and record keeping.
