Job Description
Conduct comprehensive patient assessments and create tailored care plans.
Administer medications and treatments as prescribed.
Monitor patient progress and communicate any changes to the healthcare team.
Educate patients and families on disease prevention and health management.
Work closely with physicians, therapists, and other healthcare professionals to ensure seamless patient care .
Maintain accurate, up-to-date documentation of all patient care activities.
Requirements
What We're Looking For: Active Registered Nurse (RN) license in the state of California.
Minimum of one year of nursing experience (Home Health experience required).
Strong clinical skills with a compassionate, patient-first approach .
Excellent communication and interpersonal skills to engage with patients and families.
Ability to work independently , manage time effectively, and adapt to various home environments.
Basic Life Support (BLS) certification required.
Reliable transportation for home visits within Oakdale and surrounding areas.
