Physical Therapist Home Health

Seasoned Recruitment is excited to announce an opening for a Physical Therapist in our Home Health division. This position offers a unique opportunity to deliver personalized therapy services to patients in their own homes. Our ideal candidate is dedicated, compassionate, and eager to help improve the quality of life for our clients.

Key Responsibilities:

• Conduct thorough assessments to establish patient needs and therapy goals
• Create tailored treatment plans based on comprehensive evaluations
• Administer interventions and therapies to enhance patient mobility and independence
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and their families on exercises and health strategies
• Collaborate with healthcare professionals to provide holistic care
• Comply with all regulatory requirements and best practices in home healthcare
• Full-time schedule with local travel required

Schedule:

• Weekdays: Monday to Friday, 8 AM to 5 PM
• Weekend work: One weekend per month
• No on-call responsibilities

Requirements

Qualifications:

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited institution
• Current state licensure to practice as a Physical Therapist
• Minimum of 1 year of relevant experience in home health care is preferred
• Proficiency in HCHB and OASIS documentation is highly desirable
• Strong clinical reasoning and assessment skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively as part of a multidisciplinary team
• Commitment to providing exceptional patient care
