Per - Diem Home Health Physical Therapist

Parx Home Health Care of Tampa , a leading home care agency serving Hillsborough & Pasco counties in Florida, is currently seeking a dedicated Per-Diem Physical Therapist . In this role, the Physical Therapist will work with patients in the comfort of their own homes, helping to facilitate healing and improve quality of life. The ideal candidate will have experience in home health care and be committed to providing compassionate care to patients.

Responsibilities

• Conduct patient assessments to identify treatment needs and develop individualized care plans
• Visit patients' homes to administer and monitor physical therapy treatment
• Collaborate and communicate with multidisciplinary healthcare teams
• Use appropriate tools, equipment, and techniques to provide physical therapy treatment
• Evaluate and document patient progress and adjust treatment plans as needed
• Provide patient and caregiver education on proper techniques and exercises
• Ensure compliance with all state and federal laws and regulations related to home healthcare services

Requirements

• Valid state Physical Therapy license
• Minimum of 2 years of experience as a Physical Therapist in home health care
• Strong communication and interpersonal skills
• Ability to work independently with minimal supervision
• Effective time management and organizational skills
• Flexibility to work flexible hours and travel to patients' homes
• Passionate commitment to providing compassionate, high-quality care to patients
