Nurse Practitioner - Geriatric Medicine

We are seeking a highly motivated and compassionate Nurse Practitioner (NP) to join our Geriatric Medicine team. The ideal candidate will have a passion for working with older adults and a commitment to providing high-quality, patient-centered care.

Responsibilities

• Provide comprehensive medical care to older adults, including preventive care, diagnosis and treatment of acute and chronic conditions, and management of complex medical conditions.
• Conduct thorough patient assessments, including physical exams, medication reviews, and laboratory and diagnostic test interpretation.
• Develop and implement individualized treatment plans in collaboration with patients and their families.
• Educate patients and their families about their conditions and treatment options.
• Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to provide seamless care for patients.
• Participate in quality improvement initiatives and stay up-to-date on the latest advancements in geriatric medicine.

Qualifications

• Master's degree in nursing from an accredited university
• Board certification as a Nurse Practitioner by the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)

• Minimum of 3 years of experience as a Nurse Practitioner in a geriatric setting.
• Strong clinical skills and experience in managing complex medical conditions in older adults.
• Excellent communication and interpersonal skills, including the ability to establish rapport with older adults and their families.
• Ability to work independently and as part of a team.
• Commitment to patient-centered care and evidence-based practice.

Actual pay will be adjusted based on experience and other qualifications.
