Nurse Case Manager

The Nurse Case Manager will provide help to high-risk infants and children getting health care and developmental services. The Nurse case manager will complete the following:

• promote optimal growth and development of infants.
• provide emotional support and relevant referrals to families for essential services.
• work with the mother to reduce stress and help explain identified medical conditions for further evaluation, diagnosis and treatment.
• responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High-Risk Infant follow Up Program from the State of Illinois.

• Bachelor's degree in nursing from an Accredited College or University
• Minimum of 2 years' experience working in Community Health or Maternal Child and Health nursing
• Active nurse License in the state of Illinois
