We are seeking a Licensed Master Social Worker (LMSW) to join a Review Company based in New Rochelle, NY as a Nursing Home Surveyor/Complaint Investigator . This full-time, contract role focuses on conducting nursing home surveys, investigating complaints, and ensuring compliance with federal and state regulations.

• Conduct nursing home surveys and complaint investigations, including those involving abuse or neglect
• Evaluate facilities' compliance with state and federal regulations
• Draft Statements of Deficiencies (SOD) using the Principles of Documentation
• Review and assess corrective action plans for compliance
• Complete required data entry in state and federal reporting databases
• Participate in immediate jeopardy monitoring and state oversight activities
• Provide testimony in administrative hearings as needed
• Attend required meetings and adhere to DOH and IPRO schedules and policies

Requirements

• Licensure: Active New York State LMSW license
• Experience:
• Minimum of 2 years of clinical experience in a medical facility, preferably a nursing home (required)
• 2 years of experience conducting nursing home surveillance activities (preferred)
• Certification:
• SMQT Certification (preferred; must be willing to obtain within 12 months)
• Other:
• Must be able to travel to on-site nursing home locations as required
• Strong documentation and regulatory compliance skills
