Intake Specialist

Ultimate Care NY is seeking a talented Intake Specialist to join our team!

As an Intake Specialist , your key role will be to help us educate our consumers on available services and assist them in the process to get approved. This is an exciting role that works with an Ultimate team! Join us and help us maintain our commitment to the excellence of home care services!

You will :

• Collaborate with various teams internally to ensure a smooth intake process for clients seeking home care services.
• Conduct intake assessments to determine client needs and eligibility for services.
• Educate and inform consumers about the range of care services available.
• Guide clients through the enrollment process, ensuring all necessary paperwork and requirements are met.
• Maintain accurate and up-to-date records of client information and interactions.
• Provide outstanding customer service by addressing inquiries and concerns promptly and professionally, while advocating for our patients and caregivers through the process to start home care services.
• Exhibit a positive attitude and strong teamwork skills, fostering a supportive and collaborative work environment.

You will have:

• Previous experience in home care intake coordination is preferred.

• Customer service experience in a home care environment is a plus, showcasing your ability to empathize and communicate effectively with clients and their families.
• Excellent organizational skills, attention to detail, and the ability to prioritize tasks to ensure efficient workflow.
• A proactive go-getter attitude that enables you to find creative solutions and make the most of available resources.
• A positive attitude: you are a team player, and you work effectively with different teams.
