Job Description


Accord is seeking a Housing Stabilization Specialist to join our team! You will be supporting individuals with mental and physical disabilities in the community with their personal goals and finding stable housing resources. If you this is your career path, we'd love to hear from you!

Essential Responsibilities:

  • Support adults with developmental or other disabilities, including mental illness and/or chemical dependency issues.
  • Meet with individuals in their homes and in the community.

Transitioning Services:

  • Assist individuals in identifying and securing stable housing through housing searches and landlord engagement.
  • Support individuals in completing housing applications, securing necessary documentation, addressing barriers, and understanding lease agreements.
  • Provide education on tenant rights and responsibilities.
  • Develop person-centered housing plans to guide individuals through the process from housing search to move-in.
  • Assess and address housing barriers to improve housing opportunities.
  • Advocate for individuals by negotiating reasonable accommodations and resolving housing-related concerns.

Sustaining Services:

  • Provide ongoing support to maintain housing stability through regular check-ins and crisis intervention.
  • Assist individuals in understanding and fulfilling their tenant responsibilities.
  • Implement and adjust person-centered housing plans to prevent eviction and address ongoing challenges.
  • Connect individuals to financial assistance programs, community resources, and employment supports

  • Support people to achieve personal outcomes that characterize a high quality of life.
  • Listen carefully and communicate respectfully and clearly with the people you support.
  • Support people to choose, understand and learn new skills.
  • Understand, follow through, and document support plans, goals, activities, and progress in meeting goals.
  • Work closely with individuals, case managers, landlords, and community resources to meet person centered established goals and ensure successful housing outcomes.
  • Support people to exercise their rights and responsibilities in all areas of their lives.
  • Engage in respectful communication.
  • Use person-first or preferred language when appropriate.
  • Maintain confidentiality and support privacy at all times.
  • Talk with people and promote person centered planning by encouraging them to participate in discussions about their own housing needs and lives.
  • Support people to have optimal health and well being.
  • Ensure that people are free from abuse, neglect and exploitation.
  • Complete needed reporting and documentation.
  • Provide safeguards and appropriately support challenging situations.
  • Support individuals with positive behavioral techniques to avoid challenging situations and promote respect and dignity.
  • Know the proper techniques and be able to respond to emergencies such as accidents, fires, tornados, or other situations
  • Complete documentation of all services provided in the selected electronic health record/database system within 1 business day of providing services.
  • Meet or exceed minimum billing standards of 80% of time worked.
  • Demonstrate professional communication with interdisciplinary team members including the person supported, family caregivers and guardians, employers, case managers, and other involved stakeholders.
  • Represent the agency to the people we support, our community partners and external constituents. Serve as a positive role model for others.
  • Provide transportation to individuals supported to facilitate the above responsibilities; travel throughout the Twin Cities Metropolitan area.
  • Complete all required and assigned training related to the position
  • Perform other duties as required or assigned.

Requirements

Education:

  • Bachelor's degree in human services or related field preferred.
  • If you hold a Social Work degree, we expect you to be licensed by the Minnesota Board of Social Work, as required.

Skills & Experience:

  • One year of experience working with individuals with developmental, mental health, or other disabilities required.
  • Knowledge of housing and community resource options to support individuals experiencing homelessness or at risk of homelessness preferred .
  • Excellent time-management, organizational, and people skills.
  • Demonstrated problem solving, independent judgment, strong communication and planning skills required.
