Home Recovery Care RN

The Home Recovery Care RN is responsible for providing skilled nursing care to patients in their homes. This position requires a high level of clinical expertise, compassion, and organizational skills. The ideal candidate will have a strong understanding of home health care and a commitment to providing quality patient care.

Responsibilities:

• Provide skilled nursing care to patients in their homes, including:
• Administering medications
• Monitoring vital signs
• Dressing wounds
• Providing patient education
• Developing and implementing care plans
• Collaborate with other members of the healthcare team, including physicians, physical therapists, and social workers
• Educate patients and their families on disease management and self-care
• Document patient care in accordance with company policies and procedures
• Maintain a clean and safe work environment

Qualifications:

• Registered Nurse (RN) license in the state of [state]
• 2+ years of experience in home health care or a related setting
• Strong clinical skills and knowledge of home health care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Proficiency with electronic health records (EHRs)

Additional Information:

• This position requires a valid driver's license and a reliable vehicle
• The RN will be required to work flexible hours, including weekends and holidays
