Phoenix Home Care and Hospice in Independence, MO is seeking a Home Health Scheduling Coordinator to join our team!

As a Home Health Scheduling Coordinator, you will be responsible for scheduling and coordinating the home health services provided by our dedicated team of nurses. You will work closely with the nursing staff, clients, and their families to ensure efficient and effective care delivery.

Responsibilities:

  • Create and maintain schedules for home health services
  • Coordinate and communicate with nursing staff, clients, and their families
  • Maintain accurate records and documentation
  • Troubleshoot scheduling issues and provide timely solutions
  • Ensure compliance with company policies and procedures
  • Assist with other administrative duties as needed

Requirements:

  • Prior experience in scheduling, preferably in a home health or healthcare setting
  • Excellent organizational and time management skills
  • Strong attention to detail
  • Ability to multitask and prioritize effectively
  • Excellent communication and interpersonal skills
  • Proficient computer skills and familiarity with scheduling software
  • Compassionate, caring, and empathetic
