Home Health Registered Nurse

Join Expert Healthcare as a Home Health Registered Nurse, where you can make a significant impact on the lives of patients by providing care in the comfort of their homes. In this role, you will conduct in-depth assessments, develop individualized care plans, and ensure patients receive the smooth execution of treatment programs. Your responsibilities will include monitoring vital signs, administering medications, and continuously evaluating the effectiveness of the care provided. Schedule: Monday to Friday, 8 AM to 5 PM.

• Perform comprehensive patient assessments in home settings.
• Create, implement, and adjust individualized care plans based on patient needs.
• Administer medications and treatments as per physician orders.
• Evaluate patients' conditions and document progress meticulously.
• Educate patients and families about health management and preventive measures.
• Collaborate with multidisciplinary teams to facilitate effective care.
• Provide emotional support and counseling to patients and their families.

Requirements

• Active RN license in the state of practice.
• Proficiency in using HomeCare Homebase Charting required.
• At least 1 year of home health nursing experience required.
• Strong assessment, critical thinking, and clinical skills.
• Exceptional communication and interpersonal skills necessary for patient interaction.
• Ability to work autonomously while effectively managing time and priorities.
• BLS certification required.
• Familiarity with OASIS documentation preferred.
