Responsibilities:
- Provide comprehensive physical therapy services to homebound patients
- Develop individualized treatment plans based on each patient's needs
- Educate patients and their families on proper exercises and techniques
- Collaborate with other healthcare professionals to ensure coordinated care
- Document patient progress and outcomes
- Maintain accurate and up-to-date patient records
- Comply with all applicable regulations and standards
Qualifications:
- Master's degree in Physical Therapy from an accredited university
- Current license to practice physical therapy in the state of [State]
