Home Health Physical Therapist

We are seeking a highly motivated and compassionate Home Health Physical Therapist to join our team. The ideal candidate will have a strong background in physical therapy, a passion for helping patients, and a commitment to providing exceptional care.

Responsibilities:

• Provide comprehensive physical therapy services to homebound patients
• Develop individualized treatment plans based on each patient's needs
• Educate patients and their families on proper exercises and techniques
• Collaborate with other healthcare professionals to ensure coordinated care
• Document patient progress and outcomes
• Maintain accurate and up-to-date patient records
• Comply with all applicable regulations and standards

Qualifications:

• Master's degree in Physical Therapy from an accredited university
• Current license to practice physical therapy in the state of [State]
