Expert Healthcare is looking for a compassionate and dedicated Home Health LPN to join our team of healthcare professionals. Schedule: Monday - Friday, No Weekends, No Call required.
Key Responsibilities- Provide nursing care to patients in their homes according to established care plans.
- Administer medications, treatments, and therapies as prescribed.
- Monitor and record patients' health status and report any changes to the supervising RN or physician.
- Assist with basic patient care tasks such as bathing, dressing, and mobility.
- Educate patients and families about medical conditions, medications, and care techniques.
- Coordinate with healthcare team members to ensure a comprehensive approach to patient care.
- Maintain accurate and up-to-date documentation of care provided.
Requirements
- Current and valid LPN license in the state of practice.
- Experience with HomeCare Homebase Charting preferred.
- Minimum of 1 year of nursing experience in a home health setting within the last 3 years.
- Strong clinical skills and the ability to assess patient needs effectively.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time efficiently.
- BLS certification required.
