Position Overview

We are currently seeking a Field Registered Nurse (Coordinator of Care) to join a top-tier home healthcare team serving the Bronx and Manhattan. This role involves delivering skilled nursing care, coordinating patient treatment plans, and conducting home visits, all while ensuring the highest standards of care.

Key Responsibilities
  • Conduct home care assessments to determine service eligibility
  • Develop and implement care plans in coordination with patients, families, and physicians
  • Provide direct skilled nursing care, including wound care, medication prep and administration, and disease management
  • Supervise and support home health aides
  • Collaborate with interdisciplinary teams including PT, OT, MSW, nutrition, and more
  • Document all services and updates in the HCHB system within 48 hours
  • Actively participate in case conferencing and treatment planning
  • Support discharge planning and transitions of care
  • Adhere to all infection control and compliance protocols

Requirements

Qualifications
  • Education: Associate or Bachelor's Degree in Nursing
  • Licensure: Active Registered Nurse (RN) license in New York State
  • Experience: Minimum one year of hospital nursing experience required
  • Additional Requirements:
    • Willingness to travel throughout Bronx and Manhattan
    • Strong communication and critical thinking skills
