Director of Case Management (RN, LCSW, LMSW)

The Director of Case Management is responsible for overseeing and managing all aspects of case management services within the organization. This includes providing clinical leadership, developing and implementing policies and procedures, and ensuring the delivery of high-quality, patient-centered care.

Essential Duties and Responsibilities:

• Provide clinical leadership and oversight to the case management team
• Develop and implement policies and procedures for case management services
• Ensure that all case managers are trained and competent in their roles
• Monitor the quality of case management services and identify opportunities for improvement
• Collaborate with other clinical departments to ensure the coordination of care
• Advocate for the needs of patients and families
• Stay up-to-date on current trends and best practices in case management
• Participate in research and quality improvement initiatives
• Manage the case management budget
• Prepare reports and presentations on case management activities

Qualifications:

• Master's degree in nursing (RN), social work (LCSW), or a related field
• Strong clinical and leadership skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of current trends and best practices in case management
