*2 Positions Available* Programs in Assertive Community Treatment (PACT) is an evidence-based outreach program to help people with serious mental illness integrate into the community. Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. The PACT program is a multidisciplinary team for persons with a serious and persistent mental illness. A team consists of a psychiatrist, advanced practice nurse, nurses, clinicians, co-occurring specialists, vocational specialists, mental health specialists, and peer mental health specialists. • Facilitate and complete intake process
• Complete Bio-psychosocial Assessment within 14 days of admission
• Provide services to consumers in the community through outreach
• Manage treatment plan review process, ensuring timeliness and appropriate content
• Educate staff at weekly Team Treatment Planning meetings on how to develop and engage consumer in treatment planning process
• Assist consumers with activities of daily living through modeling, direct assistance with ADLs and coaching

• Master's Degree in a Human Services field
• LAC/LSW required
• LPC/LCSW preferred
• LCADC Preferred
• Two year's experience working with individuals with serious and persistent mental illness
