Job Description


We're seeking a Case Manager to join MHA's Recovery and Housing Division, within our Housing First program.

The Case Manager supports adults experiencing homelessness or at risk of becoming homeless by assisting them in obtaining and maintaining independent subsidized housing. This role promotes advocacy, skill-building, and community engagement for program participants. Responsibilities are divided between office work and fieldwork. While the standard schedule is Monday through Friday, occasional flexibility for weekend hours is required.

Essential Functions

  1. Support for Program Participants:
    • Gathering necessary documentation for housing applications.
    • Assisting in identifying suitable housing through online applications and landlord outreach.
    • Maintaining housing through advocacy and mediation.
    • Providing referrals and connections to mental health and/or substance use providers.
    • Teaching and role modeling independent living skills.
    • Encouraging active community participation through volunteer activities.
    • Facilitating access to primary care and wellness services.
    • Supporting participants in achieving employment and educational goals.
    • Assisting with obtaining benefits such as social security, SNAP, SSI, and others.
    • Helping participants achieve sustainable income through regular employment.
  2. Additional Responsibilities:
    • Completing documentation requirements, including assessments, progress notes, and data entry.
    • Transporting program participants as needed.
    • Conducting thorough housing inspections to ensure safety and suitability.

Requirements

  • A valid driver's license, sufficient automobile insurance, an acceptable driving record, and a safe, reliable vehicle during working hours.
  • The ability to work independently, manage time effectively, prioritize tasks, take initiative, and collaborate as part of a team.
  • Strong oral and written communication skills, as well as computer and organizational proficiency.
  • Adaptability to the changing needs of program participants.
  • Patience, compassion, and strong interpersonal skills.
  • The ability to obtain and maintain certifications in medication administration (MAP), CPR, and first aid (training provided).
  • A minimum age of 18 years old.

Education and Experience

  • A high school diploma or equivalent (GED).
  • Experience working with individuals with disabilities.
  • (Preferred) Experience assisting individuals with housing-related needs.
  • (Preferred) Bilingual abilities.
  • Compassion and empathy, with a strong desire to assist others.

Physical Demands

  • Frequent talking, hearing, sitting, typing, and writing.
  • Occasional kneeling, climbing stairs, or lifting objects up to 40 pounds.
  • Visual perception for inspection and coordination tasks.
