Caregiver (PRN)

Overview Heritage Home Healthcare is seeking compassionate and dependable PRN caregivers in the Rio Rancho and Bernalillo, NM area to provide delegated personal care services in a home setting. In this role, you will support clients with activities of daily living, ensuring comfort, safety, and dignity. This flexible, on-call position is ideal for individuals who are reliable, observant, and passionate about providing quality in-home care.

Coverage Area ZIP Codes: 87144, 87004, 87124, 87174

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and eating
• Provide mobility assistance, including transfers and safe positioning
• Support medication reminders and delegated care tasks per care plan and state regulations
• Assist with light housekeeping and maintaining a safe home environment
• Provide companionship, emotional support, and encouragement
• Escort or transport clients to appointments or errands as needed
• Observe and report changes in client condition to the clinical team
• Follow all individualized care plans and instructions from licensed professionals

• Compassionate, reliable, and patient caregivers
• Strong communication and time-management skills
• Ability to work independently in a home setting
• Willingness to travel between client homes in the assigned area
• Commitment to high-quality, client-centered care

• Must be at least 18 years of age
• Previous caregiving experience preferred
• Ability to follow care plans accurately and independently
• Strong interpersonal and communication skills
• Valid driver's license and reliable transportation
