What does a Care Manager do?

Job Description

We are looking for an understanding care manager to create individualized care plans for patients. This person will make sure that all healthcare providers are informed about the patient's needs and work closely with their families, friends, and caregivers to share important information. The care manager will also build relationships with high-quality healthcare providers and keep accurate records of patient care. They will also help connect patients with community programs and services, like housing assistance and self-care programs.

To be successful in this role, you should have good knowledge of the healthcare system and be able to build strong relationships with healthcare providers. Ideal candidates will also be able to handle multiple tasks in a fast-paced environment and have strong negotiation skills.

Job Duties and Responsibilities

  • Create and update personalized care plans for patients.
  • Work with patients' families, friends, and support systems to develop treatment plans.
  • Find and recruit high-quality healthcare providers.
  • Communicate with healthcare providers to manage patients' treatment.
  • Make sure patients' medical needs are being met.
  • Suggest alternative treatment plans if needed.
  • Teach patients about procedures, healthcare instructions, and referrals.
  • Connect patients with social services programs, like transportation assistance and translation services.
  • Follow up with patients regularly to check on their progress and make sure they are receiving proper care.
  • Keep records of all case management activities.

Requirements and Qualifications

  • Bachelor's degree in social sciences, social work, nursing, or a related field.
  • At least 2 years of experience in public health or a related field.
  • At least 1 year of experience in case management.
  • Registered Nurse (RN) license.
  • Certified case manager (CCM) license preferred.
  • Proficient in Healthcare Management Systems and Microsoft Office Word and Excel.
  • Excellent written and verbal communication skills.
  • Strong organizational skills.
  • Ability to travel locally.
